Study: Many parents don't see obesity

 

 

 

DETROIT - A startling number of parents might be in denial about their youngsters' weight.

A survey found that many Americans whose children are obese do not see them that way.

That is worrisome because obese children run the risk of diabetes, high blood pressure, cholesterol problems and other ailments more commonly found in adults. And overweight children are likely to grow up to be overweight adults.

"It suggests to me that parents of younger kids believe that their children will grow out of their obesity, or something will change at older ages," said Dr. Matthew M. Davis, a University of Michigan professor of pediatrics and internal medicine who led the study, released earlier this month.

Among parents with an obese, or extremely overweight, child ages 6 to 11, 43 percent said their child was "about the right weight," 37 percent responded "slightly overweight," and 13 percent said "very overweight." Others said "slightly underweight."

For those with an obese child ages 12 to 17, the survey found more awareness that weight was a problem. Fifty-six percent said their child was "slightly overweight," 31 percent said "very overweight," 11 percent said "about the right weight" and others said "slightly underweight."

Dr. Goutham Rao, clinical director of the Weight Management and Wellness Center at Children's Hospital of Pittsburgh, said obesity in children isn't as easy to identify as in adults. "Plus, because of the social stigma, it's not something that parents are willing to admit to readily," Rao said.

The survey of 2,060 adults, conducted over the summer by Internet research firm Knowledge Networks, used children's height and weight measurements collected from their parents to calculate body mass index.

When a child's BMI was higher than the 95th percentile for children who are the same age and gender, the child was seen as obese.

Based on what the parents reported, 15 percent of the children ages 6 to 11, and 10 percent of the children ages 12 to 17, were obese.

The Michigan researchers said that, too, suggests parents underestimate their children's weight. National estimates indicate about 17 percent of U.S. children are obese under the standard used by the researchers.

America's obesity myth?

If you're like most Americans, you've probably stuffed yourself like a holiday turkey during the past few weeks. So it should come as no surprise that the average American gains about one pound between Thanksgiving and New Year's, according to the National Institutes of Health. That pound a year really adds up over the decades. Today, 17 percent of children are overweight. By the time they reach adulthood, that number climbs to 66 percent.
 
It's a common political refrain that America faces a childhood obesity epidemic that's turning us into a nation of blubbery diabetics.

Underlying this is the premise that we're helpless before gingerbread cookies and honey-roasted hams -- unable to resist these and other foods and incapable of putting down our forks. We can be cured, it seems, only by government intervention such as the banning of trans-fats and sodas from public schools.

But is it the food, or is it us? Is it a proper role of government to tell us what we can or can't eat? And are we really as fat as the NIH numbers suggest? Before we let Uncle Sam into our kitchens these questions deserve some exploration.

For starters, government data about what constitutes "overweight" and "obese" are misleading.

The standard metric for this measurement is a person's body-mass index, or BMI -- the ratio of one's height to one's weight. But at best, BMI is a rough tool that does not take into account an individual's body type. A 6-foot-2 athlete who weighs 210 pounds would be classified as "obese" according to BMI charts -- despite his less than 6 percent actual body fat.

If you believe the BMI tables, most of the best players in the NBA and NFL are "overweight," including superstar athletes Kobe Bryant and Tom Brady. Many Hollywood heartthrobs also qualify as fatties -- Brad Pitt, Matt Damon, Tom Cruise and George Clooney, to name a few.

What's more, the acceptable BMI continues to be ratcheted downward -- transforming those who were considered perfectly healthy yesterday into "overweight" and "obese" today.

Before 1998, a "healthy" BMI was anything less than 27. Then, suddenly, the government changed the "healthy" number to anything less than 25. Overnight, more than 25 million people who were previously considered to be a healthy or normal weight were reclassified as overweight.

This raises the question: Are supposedly overweight people in fact heavier than they ought to be?

While more people might be overweight now, and it's true that people who are seriously overweight tend to have a higher risk of developing some illnesses, the government flip-flop suggests how difficult it is to determine what truly is overweight.

Even the Centers for Disease Control and Prevention had to publicly concede in 2005 that its estimate a year earlier of "400,000 obesity-related deaths per year" should have been 112,000. But once prevented deaths are factored in, the figure is closer to 26,000 deaths per year -- one-fifteenth its original estimate.

The President's Council on Physical Fitness and Sports has stated that what matters most, in terms of overall health, is whether a person is active. People who happen to be a few pounds overweight but who exercise regularly "have a lower morbidity and mortality than normal weight individuals who are sedentary."

More important, is it government's role to help us reduce our rolls? Or is it a matter of personal responsibility?

We know that fries and cheeseburgers aren't healthy fare. And thanks in part to heightened concerns about obesity, we can now buy low-fat salads at just about every fast-food outlet in the country.

The same supermarkets and convenience stores that sell popcorn and candy bars also sell healthful foods.

People make choices. And government should protect -- not restrict -- the freedom to make those choices so long as we're not harming others.

While we may not always like the choices others might make, it is essential that we all have the freedom to choose for ourselves. Once we accept the idea that the Nanny State should step in when it's "for our own good," we've taken a very big step down the road to something like the scene painted in George Orwell's "1984" -- when citizens wake each day to mandatory exercise classes on the Telescreen.

Most of us would prefer to choose for ourselves whether to exercise or have an extra helping of apple pie. And if we gain an extra pound over the holidays -- so what? That's why we have New Year's resolutions.

Increased consumption of beverages is contributing to increased calorie intake. In the US in 1965 beverages accounted for just 12 percent of daily energy intake but by 2002, this number had jumped to 21 percent. This increase in consumption is not being offset by a reduction in calories from food.

Across the whole Asia Pacific region, over two thirds of consumers were trying to lose weight even though only half considered themselves overweight. In South Korea, which has the lowest levels of obesity, 90 percent were trying to lose weight. In Asia, consumers are more likely to control weight through diet than exercise.

Combating childhood obesity

The new year is approaching quickly and it's never too early to consider nutrition and exercise, especially for children and teens.

In Henrico County, public school school students already exercise to loud music and video routines while at school. The students' heart rates are monitored by devices worn on the wrist while exercising. The monitors allow students to self-pace their workout and target the appropriate heart rate.

The program is about "getting kids to take ownership of their own fitness, but through a method that they would be attracted to," said Bonnie Conner-Gray, an education specialist in Henrico.

According to the Centers for Disease Control and Prevention's Web site, "Overweight children and adolescents are more likely to become overweight or obese adults; one study showed that children who became overweight by age 8 were more severely obese as adults."

Obesity Is in the Brain - Brain hormone linked to hunger control

Western world is experiencing an obesity pandemics, triggered by high-calorie content food, overeating and sedentariness. A new research published in "The Journal of Neuroscience" and carried on by a team led by Dr. Maribel Rios, an assistant professor of neuroscience at the Sackler School of Graduate Biomedical Sciences, at Tufts University School of Medicine in Boston, has found for the first time a chemical involved in overeating: a protein named brain-derived neurotrophic factor (BDNF).

Mice engineered to lack Bdnf gene in two of the primary appetite-regulating brain areas ate more and turned overweight.
"We knew that the global lack of BDNF and/or its receptor during development leads to overeating and obesity in young mice. However, it remained unclear and controversial whether BDNF mediated satiety in adult animals. Our recent findings demonstrate that BDNF synthesis in the ventromedial (VMH) and dorsomedial hypothalamus (DMH) is required for normal energy balance. We were able to establish that BDNF acts as a satiety signal in the mature brain independently from its putative actions during development of the brain. This important distinction might help define disease mechanisms and critical periods of intervention for the treatment and prevention of obesity disorders", said Rios.

The engineered mice turned obese just through overeating.

"Normal body weight was restored in mutant mice when food access was limited to that of normal mice, indicating that deletion of the Bdnf gene in the VMH and DMH does not affect the expenditure side of the energy balance equation", said Rios.

The team also determined the levels of BDNF synthesis connected to nutritional status.

"The amount of BDNF mRNA produced decreased during periods of fasting. However, when the mice were exposed to glucose, a macronutrient, we observed a rapid, but transient, increase in the expression of BDNF and its receptor. These changes occurred specifically in the VMH, which is known to be involved in the regulation of food intake", said first author Thaddeus Unger.

Thus, glucose has a direct impact in the brain, controlling the hunger sensation.

"Direct administration of BDNF into the brain also led to an immediate increase in the levels of an early-response gene and marker of nerve-cell activation in both the VMH and the DMH. These results suggest that BDNF is a fast-acting signal inducing neuronal activity within neural circuits involved in appetite control. Mice with site-specific perturbation of BDNF expression did not exhibit behavioral abnormalities typically observed in mice with global deletion of the Bdnf gene throughout the brain, such as hyperaggression, depressive-like behavior, and hyperactivity. The absence of these behaviors suggests that BDNF expression in the VMH and DMH is not required for regulation of non-appetite-related behaviors. It appears that this signaling pathway acts, at least partly, through short-term mechanisms and that BDNF synthesis in the VMH and DMH is required for suppression of appetite", said Rios.

"This work brings us closer to elucidating the brain pathways that rely on BDNF to modulate food intake. The relevance of the BDNF signaling pathway in human disease is highlighted by the obesity exhibited by certain humans carrying mutations or abnormalities in the genes coding for BDNF or its receptor. This is bound to be an important area of obesity research as more than a quarter on the American population has been estimated to carry mutations in the Bdnf gene", she added.

Tax sodas to fight obesity says mayor of San Francisco

 

 

 

The mayor of San Francisco, in an attempt to improve the overall health of citizens, is considering a tax on sodas and other drinks which are artificially sweetened by high-fructose corn syrup.

Mayor Gavin Newsom plans to explore the possibility of imposing a small tax on retailers who sell such beverages.

Newsom says beverages sweetened by high-fructose corn syrup go against all efforts to combat obesity.

He says there is mounting evidence that children are increasingly at risk for poor health because of such drinks, and feels it is time to 'explore every avenue and examine every innovative idea, in the hope of making obesity a thing of the past'.

Apparently apart from regulating citizens' health and fighting the growing problem of obesity, the tax would also be used to further extend Shape Up San Francisco, a city program aimed at encouraging people to engage in exercise, healthy diets, and other similar preventive practices.

The initiative would be a response to findings by the state's public health department, which have revealed a 500 percent increase in the number of calorically sweetened beverages in the past 50 years.

These drinks have been identified as one of the leading sources of excess sugar in children's diets and the increase in the consumption of these drinks is linked to childhood obesity - a link that is much stronger than other identified factors, such as high fat content, or decreased physical activity.

Soft-drink makers are of course not enamored with the idea and say the mayor's plan is a "flawed strategy" that would have little effect and obesity is a complex issue with no single cause.
